I understand that participation in a retreat organized by The Open Table, a nonprofit organization, includes inherent risks – including, but not limited to: physical activity, emotional reflection, travel, and the use of indoor and outdoor facilities.
I assume full responsibility for my participation and voluntarily release The Open Table, its board, staff, volunteers, agents, and any retreat host sites from any and all liability, claims, or expenses arising from injury, illness, damage, or loss.
I have read and agree to the Assumption of Risk and Release of Liability, as indicated by my signature below.
In the event of illness or injury, I authorize The Open Table staff and/or volunteers to administer first aid and seek emergency medical care on my behalf if I am unable to do so. I understand that I am responsible for any associated costs.
I give consent for emergency medical treatment if needed, as indicated by my signature below.
I affirm that I am healthy enough to participate in retreat activities. I agree not to attend if I am sick, contagious, or test positive for COVID-19 within 5 days of the event. I acknowledge the risk of COVID-19 exposure and accept it voluntarily.
I accept the health risks, including potential COVID-19 exposure, as indicated by my signature below.
I give permission for photographs or videos taken of me at the retreat to be used by The Open Table for nonprofit promotional or educational purposes. My name will not be used without my permission.
I confirm that my participation is voluntary, and I agree to all of the terms above. I understand that this form is a legally binding agreement.
